I, the undersigned, as the participant or the parent/legal guardian of the minor participant, do hereby give my full consent and approval for my child to participate as a member of SUDBRINK PERFORMANCE ACADEMY activities related to the rehearsal and performance of DESCENDANTS THE MUSICAL. I understand that there are risks of damages and injuries, including death, inherent in any activity, and I am willing to assume these risks and not hold SPA liable for those damages or injuries. I understand the new precautions that SPA personnel have in place to protect participants from the spread of illness. I have read and agree to all changes and new protocol in place by SPA. I further agree that I shall hold harmless and fully indemnify the parties hereby released from any and all claims, damages, costs and expenses, including attorney fees and medical fees, arising from any action made by me or any persons or entities hereby released. *

I have read and understand this waiver, release of liability and indemnification agreement, I understand the terms of the agreement, and I agree to abide by them.

Photo/Video Consent We ask your consent for still photographs and video production to be taken of you (or your child, if a minor), by SPA or photographers arranged to document the events. Images or video are for use of SPA only, for displays, website, marketing, advertising, and archival purposes, and will never be sold. I consent for photography and videos to be used for publicity and marketing of Sudbrink Performance Academy. *
